Welcome to our practice! We are happy that you have chosen us for your dental care. To help prepare you for your first appointment, we have put together some notes for what to expect.
For General/Family Dentistry Appointments:
Your first appointment will include a new patient examination by Dr. Leung who will evaluate your general dental needs, followed by a hygiene cleaning by one of our dental hygienists.
You will need to arrive at least 15 minutes before your first scheduled appointment to fill out a medical history form and give our receptionist your dental insurance information. If you wish to save some time, you can download and fill in these forms before you arrive:
If you have dental insurance, we ask that you provide the following information on your first visit:
- the name of your insurance provider
- the group policy number
- the Certificate or ID number
- the name and date of birth of the policy holder
- a copy of the benefit booklet
For Prosthodontic Appointments:
You need the care of a prosthodontist if:
- You are missing one or more teeth.
- You are interested in dental implants.
- You wear dentures or removable partial dentures.
- You want to improve the esthetics of your smile.
It is no small task to consider prosthodontic care. Whether you are referred by your family dentist or you are self-referred, you will probably have many questions in mind. Dr. Leung understands your concerns and will take the time to answer all your questions.
Below are the steps in getting started:
Your first appointment will take approximately 45-60 minutes during which time Dr. Leung will provide an initial assessment of your dental needs and concerns. You will be given an approximate time frame and cost of treatment. If your family dentist has referred you to our practice, we ask that you bring the referral form and any current x-rays to the first appointment.
Then, if you agree to the prescribed preliminary treatment plan outlined in your initial consult, we will require one to two more appointments. Dr. Leung will need to perform a comprehensive examination of your mouth, including any necessary radiographs, extra-oral and intra-oral pictures, as well as impressions of your current dentition. Depending on the complexity of the treatment, you may require another appointment to prepare and finalize your treatment plan and estimate.
We will go over everything in detail and answer any questions you may have. After review of the treatment plan, we can arrange for appointments for the work and a payment plan that is convenient for you before the work begins.
